Q: Do I need to bring anything to my appointment?
A: Yes. On your first visit, you will need to bring your insurance card, state ID or driver’s license and co-payment (as applicable).
Q: How long should I plan for my appointment?
A: On average, a patient should plan for 1-2 hours for the first appointment. We typically ask patients to arrive 15 minutes early to complete their sign-in and registration.
Q: How should I request medication refills without an appointment?
A: Medication refills are requested without an appointment as long as you have been seen with in the last six months. To request, call our nurses line 615.254.9981 option 2. A medication request without an appointment may take up to 24 to 48 hours.
Q: Who should I follow up with regarding my specialist referral ?

Q: How may I schedule an appointment?

Q: If I am having an issue with my services who do I contact?

We work with most major insurance providers and do our best to provide stress-free paperwork. Our practice will bill auto insurance if you have an injury due to an automobile accident. If you are in doubt about whether we accept your plan, simply contact us.
